Here is uploaded archive with icons for Visual Studio 2012/2013 templates. It's included pre-rendered *.png files and *.psd source. Icons based on standard elements of Visual Studio template icons.

No, action icons are not missing. It's default icons for C#, VB and C++/CLI class library. You already use it, see your project templates. For example, it's code from C# custom action *.vstemplate file:

<Icon Package="{FAE04EC1-301F-11d3-BF4B-00C04F79EFBC}" ID="4547" />

All that you need to do, it's detect GUID and ID for C++/CLI class library icon.

Finally got things straightened out between OuterCurve and DocuSign. My resignature is also completed on my side.

This is now merged. The C++ custom action project icon is the old style because Microsoft left its icon external.
